We are seeking an experienced and compassionate Care Team Lead (RN or PharmD) to join our growing clinical operations team. This is a leadership role that combines people leadership, clinical excellence, and operational ownership.
You will lead and mentor a team of Registered Nurses (RNs) and Clinical Pharmacists (PharmDs). This role reports to the Clinical Care Specialist Manager and plays a critical role in ensuring high quality, safe, efficient, and scalable care delivery across our cardiometabolic programs.
Team Leadership & Supervision
Daily Clinic Support
Lead and manage a team of PharmDs and RNs to deliver personalized and evidenced based care.
Provide clinical guidance to the team on:
Medication side effects, contraindications, and interactions
Dose adjustments and medication switches
Triaging and escalation decisions
Facilitate team huddles to share updates, address concerns, and escalate issues to the appropriate point of contact for resolution.
Monitor daily staffing levels to ensure adequate coverage and high quality care delivery.
Collaborate with schedulers and leadership to mitigate staffing gaps or surges in demand, proactively addressing potential challenges.
Address member concerns and escalate safety occurrences.
Team Support and Leadership
Mentor team members, fostering professional development and ensuring high quality patient care.
Synthesize immediate and long term needs of the Care Team to clinical leadership.
Conduct meetings direct reports to provide feedback, address concerns, and foster professional growth.
Provide timely feedback regarding performance and or quality issues.
Performance Monitoring
Conduct regular reviews of the team’s performance metrics and ensure adherence to SLAs and SOPs while optimizing schedules to maximize efficiency for the clinical team.
Ensure the team adheres to clinical guidelines, regulatory standards, and company protocols for weight management and medication therapy.
Assist in creating and implementing quality improvement opportunities.
Operational Excellence
Participate in and operational projects, including creating and refining workflows, policies, and procedures in collaboration with the Clinical Care Specialist Manager.
Support initiatives to improve clinical operations and team efficiency, ensuring a constant feedback loop and path to resolution.
Communication & Collaboration
Provide operational support to the Clinical Care Specialist Manager.
Serve as a liaison between the clinical team and leadership, ensuring clear communication and effective issue resolution.
Partner with the training team to ensure a seamless staff transition from training.
Collaborate with other interdisciplinary teams within the company, including clinicians, client success, product, and other cross functional team members.
Clinical Care:
Provide direct patient care, as needed.
Minimum Qualifications
Registered Nurse (RN) or Doctor of Pharmacy (PharmD)
Active license in good standing (at least in your home state)
5+ years of clinical experience in nursing or pharmacy, preferably in chronic disease management, weight management, cardiometabolic care, or related fields
Comfort practicing clinically in a telehealth / virtual care environment
Experience working in fast-paced, operationally complex clinical settings
Skills
Strong clinical judgment and patient centered approach to care
Excellent communication skills (written and verbal)
Ability to balance hands on clinical work with team leadership responsibilities
Highly organized, operationally minded, and comfortable in a scaling environment
Preferred Qualifications
Prior work at a digital health company focused on chronic conditions.
1 year in a leadership or supervisory role is preferred.
Compensation
Compensation for this role is based on credentials, licensure, and relevant experience. The expected base salary range for this position is up to $95,000 annually for Registered Nurse (RN) candidates and up to $140,000 annually for PharmD candidates.
Actual compensation within this range will be determined by a variety of factors, including but not limited to education, years of experience, geographic location, internal equity, and business needs.
In addition to base compensation, this role is eligible for a comprehensive benefits package, which includes health, dental, and vision insurance, paid time off, holidays, and other company-sponsored benefits, subject to plan terms and eligibility.
