We’re hiring a Care Navigator Team Lead to oversee and mentor a team of care navigators while serving as a key point of contact for patients. You will ensure patient onboarding, scheduling, and follow-ups are executed with precision, compassion, and operational efficiency. This role combines hands-on patient support with leadership responsibilities, including coaching, performance tracking, and process improvement initiatives.
