The Population Health Manager is a collaborative, strategic clinical leader who helps drive the success of Aledade’s value-based care initiatives by championing health equity and leveraging data-driven insights. This role specifically focuses on high-impact, low-performing outlier practices that have a unique opportunity to improve the population health of their communities by addressing health equity. This role facilitates high-level change management, performance improvement, and the implementation of specialized programs, such as those related to Social Determinants of Health (SDOH).
The Manager is an innovative self-starter responsible for developing new population health initiatives and forging external partnerships to secure resources for patients. This role must gain credibility with multiple stakeholders in order to facilitate the implementation of strategic plans that optimize patient outcomes.
This position requires 50% travel to partner practices to conduct in-depth assessments and facilitate change.
Primary Duties:
- Change Management & Strategic Implementation:
- Develop and execute strategic change management plans for new, complex population health initiatives.
- Lead discussions with practice leadership to identify performance barriers and co-create innovative, practice-specific solutions.
- Manage and own the "Gemba Walk" process, conducting comprehensive on-site assessments and developing strategic action plans to drive practice transformation.
- Serve as the primary subject matter expert for the implementation of new population health programs and initiatives across practices.
- Performance Improvement & Data Utilization:
- Lead the analysis of complex practice performance data to identify trends and opportunities for improvement in quality metrics, patient engagement, and cost efficiency.
- Research and analyze public health data and community-specific challenges that impact a practice's ability to achieve value-based care (VBC) outcomes.
- Translate high-level data and analytics into actionable, strategic insights for practice leadership and staff.
- Drive the implementation and management of specialized Social Determinants of Health (SDOH) solutions at the point of care to address health equity.
- Work directly with local hospitals and health plans to secure critical resources for under-resourced patients.
- Relationship Management & Collaboration:
- Build and maintain strategic partnerships with both internal Aledade teams and practice leadership to ensure seamless integration of new population health resources and initiatives.
- Design and deliver advanced training programs for practice staff on strategic workflows and the effective use of Aledade's population health tools.
Minimum Qualifications:
- Bachelor’s degree in relevant field required
- Clinical license in good standing: RN, Pharmacist, APRN, PA
- 5+ years of experience in healthcare, with a strong focus on practice management, quality improvement, or population health
- Proven experience in change management and strategic implementation of new programs, Certification in Change Management (e.g., PROSCI, Lean Six Sigma)
Preferred Qualifications:
- Master's degree in Public Health, Healthcare Administration, or a related field preferred
- Expertise in data analysis and ability to translate data into actionable insights for diverse audiences and drive data-informed decisions
- Deep understanding of value-based care, health equity and Social Drivers of Health (SDOH)
- Exceptional communication and relationship-building skills, with the ability to influence and collaborate with practice leadership
- Excellent communication, interpersonal, and presentation skills
- Knowledge of value-based care models (e.g., MSSP, Medicare Advantage) and quality metrics
- Experience with electronic health records (EHRs) and population health management tools
- Must be results-oriented with high adaptability in dynamic healthcare settings and the ability to support multiple projects simultaneously and autonomously
- Strong intuition for exercising discretion and confidentiality
Physical Requirements:
- Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required.
- Ability to drive to partner practices. Drivers license in good standing.
- Ability to travel 50%+ of the time to partner practices
