BillionToOne is seeking a Senior Manager of EMR & Interoperability to own the strategy, execution, and scaling of our EMR connectivity across the business. This leader will enterprise the EMR function—building a high-performing team of analysts and project/program managers, standardizing intake and delivery, and ensuring reliable, compliant, and scalable integrations that improve provider workflows and accelerate order volume growth.
This role sits at the intersection of healthcare interoperability, operational excellence, and cross-functional leadership. The ideal candidate combines deep expertise in HL7/FHIR and EMR integration ecosystems with strong program leadership, vendor management, and a track record of scaling integration operations in a fast-growing healthcare environment.
Responsibilities:
- EMR Strategy & Operating Model
- Define and execute BillionToOne’s enterprise EMR interoperability strategy across business units, aligned with growth targets and customer needs.
- Establish the operating model, standards, and “golden paths” for EMR delivery (intake, prioritization, implementation, validation, go-live, and post-launch support).
- Build a scalable roadmap to increase integration throughput while reducing time-to-go-live and operational burden.
- Develop executive-level metrics and reporting for EMR performance (volume impact, delivery velocity, reliability, and customer satisfaction).
- Integration Delivery & Program Leadership
- Lead end-to-end execution of EMR integrations across the portfolio, ensuring predictable delivery, strong communication, and stakeholder alignment.
- Own intake and prioritization of new integration requests in partnership with Sales, Product, Clinical Ops, and Engineering.
- Standardize project governance across integrations (RACI, timelines, risk management, RAID logs, go/no-go readiness).
- Drive adoption of repeatable integration patterns (hub-based approaches, templates, automation) to scale delivery.
- Integration Operations & Reliability
- Ensure accurate and timely transmission of orders and results across partner systems, with appropriate controls and monitoring.
- Oversee HL7/FHIR connectivity via Redox and other vendors, including implementation, testing, validation, and ongoing monitoring.
- Own triage and resolution processes for integration issues (missing/misrouted orders/results, compendium/configuration errors, interface failures), ensuring clear SLAs and escalation paths.
- Reduce manual touchpoints through automation (order association/matching, exception handling, monitoring) and continuous process improvement.
- Vendor & Partner Management
- Manage strategic relationships with EMR vendors, integration partners (e.g., Redox), and implementation teams to improve integration capability and customer experience.
- Negotiate and manage vendor performance against SLAs, delivery timelines, reliability targets, and roadmap commitments.
- Partner directly with customer hospital/clinic IT teams to accelerate implementation and resolve technical or workflow barriers, maintaining a strong customer-first mindset and prioritizing an excellent customer experience throughout.
- Cross-Functional Leadership
- Serve as the executive point of contact for EMR interoperability across IT, Engineering, Product, Sales, Legal, and CS
- Partner with Sales and Customer Success to ensure smooth onboarding, including IT questionnaires, technical discovery, BAAs, and go-live readiness.
- Collaborate with Product and Engineering to influence platform investments that improve scalability, integration tooling, and self-service enablement.
- Ensure documentation, playbooks, and training are consistently maintained and adopted across teams.
- Team Leadership & Scaling
- Build, mentor, and lead a high-performing EMR team (analysts, integration specialists, program/project managers), setting a strong culture of accountability and operational excellence.
- Establish clear roles, career paths, performance expectations, and operating rhythms to scale output and quality.
- Drive continuous improvement across people, process, and technology to support rapid growth in clinic integrations.
Qualifications:
- 7+ years of experience in EMR integrations, interoperability, healthcare IT, lab systems, or related domains.
- 5+ years leading teams and scaling cross-functional programs (people leadership + program ownership).
- Deep hands-on expertise with HL7 v2, FHIR, and integration engines/platforms (e.g., Redox, Mirth, etc.).
- Proven track record delivering and scaling multiple EMR integrations (Epic Aura, Cerner/Oracle, Athenahealth, eClinicalWorks, NextGen, etc.) with measurable operational and business impact.
- Strong program management skills: prioritization, governance, stakeholder communication, risk management, and execution at scale.
- Experience partnering with Engineering/Product teams on integration tooling, automation, monitoring, and scalability improvements.
- Strong understanding of privacy/security requirements in healthcare (HIPAA; preferred familiarity with SOC 2 and audit-ready processes).
- Bachelor’s degree in Health Informatics, Information Systems, Engineering, or related field preferred (Master’s a plus).

For this position, we offer a total compensation package of up to $309,665 per year, including a base pay range of $168,961 - $195,361 per year.
